JOB DESCRIPTION

Clinical Pharmacist in a hospital setting plays a key role in optimizing medication therapy to ensure safe, effective, and cost-efficient drug use for patients. Their role extends beyond dispensing medications — they are integral to the healthcare team, providing direct patient care and collaborating with doctors and nurses.

Key Responsibilities:

  • Review and validate medication orders for appropriateness, drug interactions, allergies, and dosage.
  • Provide drug information and therapeutic recommendations to physicians and other healthcare professionals.
  • Monitor patient responses to therapy and adjust medications as needed.
  • Participate in patient care rounds (especially in ICUs or specialized wards).
  • Ensure compliance with hospital policies, government regulations (FDA, DOH, etc.), and accreditation standards (e.g., PhilHealth, JCI).
  • Educate patients on proper medication use, especially those on long-term or complex therapies.
  • Help develop and update hospital formulary and therapeutic guidelines.
  • Assist in pharmacovigilance and report adverse drug reactions (ADR).
  • Support antimicrobial stewardship programs and other clinical protocols.

Requirements

Educational Background:

  • Must have a Bachelor of Science in Pharmacy (BSPharm).
  • Preferably holds a Doctor of Pharmacy (PharmD) degree (becoming more common in advanced hospital roles).


Preferred Experience & Skills:

  • Previous experience in hospital or clinical pharmacy (1–2 years minimum for some institutions).
  • Knowledge of pharmacokinetics, drug interactions, and evidence-based medicine.
  • Familiarity with hospital information systems (e.g., electronic health records).
  • Strong communication and counseling skills.
  • Certification or training in clinical pharmacypharmacotherapy, or antimicrobial stewardship is a plus.
  • Participation in continuing professional development (CPD) as required by PRC.


Licensure:

  • Must be a licensed pharmacist in the Philippines, registered with the Professional Regulation Commission (PRC).
